Job Summary
Implement treatment plan goals and objectives in collaboration with caregivers, youth, and the treatment team. Advocate for resources supporting youth and family behavioral health needs while coordinating services with agencies such as DMH, DCF, DYS, DPH, and MCI. Participate in Care Planning Team meetings for Intensive Care Coordination (ICC) cases and maintain timely, accurate documentation and data entry per payer standards. Provide culturally responsive, trauma-informed services and engage in supervision, team meetings, and quality improvement activities. This full-time role (40 hours/week) includes availability between 2–8 pm Monday through Friday to meet with families. Candidates must possess an associate's or bachelor's degree in human services, one to two years of experience with children and adolescents, and a valid driver's license.
Required Qualifications
- Associate's and/or Bachelor's degree in a human services field
- 1–2 years of experience working with children, adolescents, and transition-age youth
- Experience developing and facilitating training programs for families and/or staff
- Valid driver's license
- Reliable car
- Ability to follow written and verbal directions effectively
- Strong communication skills
- Strong documentation skills
- Commitment to equity, cultural responsiveness, and family-centered care
- Vaccination for influenza
- Vaccination for Covid-19
- Availability between 2 – 8 pm Monday through Friday
