The Recovery Place Mental Health Case Manager

Conduct assessments of individuals' backgrounds to identify needs and determine service eligibility. Develop plans to address individual needs by networking with community agencies and coordinating resources for short-term or long-term support. Provide services for a designated case load, including initiating and attending individual and group meetings, facilitating skill development, and providing transportation according to identified plans. Complete and submit documentation regarding services provided while evaluating individual progress against measurable goals. This 24-hour facility at The Recovery Place serves adult clients with multi-faceted mental health needs through collaboration with clinicians, medical staff, and direct care teams.
