Job Summary
Position coordinates community-based prevention for at-risk youth and families within the FCCP, developing and facilitating wraparound service plans, collaborating with Juvenile Justice System and multiple community providers to reduce youth involvement in Juvenile Justice; travel within the community, work evenings/weekends, lift up to 20 lb, and maintain confidentiality; requires a degree (associates or bachelor’s strongly preferred), certification pathways (Community Health Worker; Wraparound process) within 12 months, and abilities in bilingual/multilingual communication; strong interpersonal, organizational, and customer service skills are essential.
Required Qualifications
- Associate's or Bachelor's degree strongly preferred
- Ability to obtain Certified Community Health Worker certification within 12 months
- Ability to become certified in the Wraparound process within 12 months
- Experience working with at-risk youth and families with complex needs
- Experience working with culturally diverse communities/families
- Strong interpersonal and written/oral communication skills
- Ability to provide services in youth home and community locations
- Maintaining valid driver's license, registration and auto insurance
- Flexibility to work evenings and weekends as needed
- Bilingual/ASL skills and multilingual skills are compensated by an additional 6-8% above base pay
