Job Summary
The Trauma Systems Therapy- Community Health Worker will conduct screenings and individual needs assessments in family homes and other community venues, engage clients, implement screening tools, identify areas of need, develop action plans, ensure referrals to matched services, provide Enhanced Case Management to help families/youth access services, and advocate for youth and families navigating the child welfare system. The role requires CHW certification, flexible scheduling (including some evenings), potential bilingual capabilities (Spanish preferred; ASL/multilingual skills valued with compensation), and driving to client homes and community sites within Rhode Island. Physical duties include travel and lifting up to 20 lbs; training to achieve certification within 6 months; and collaboration within an interdependent team to deliver services statewide.
Required Qualifications
- Community Health Worker Certification required
- Bachelor degree preferred
- Availability to work flexible schedule including 2-3 late evenings weekly
- Ability to complete training and become certified as a Community Health Worker within 6 months
- Spanish bilingual preferred
- Valid driver’s license, auto insurance and reliable transportation
- Ability to work independently and as part of a team
- Ability to provide services in clients’ homes, health clinics, community locations and other agency sites
- Flexibility to include some evenings
- Prior experience in behavioral health and/or crisis intervention preferred
- Bilingual/ASL compensation percentages
