Job Summary
Conduct psychosocial assessments, discharge planning, and counseling for patients under the supervision of a Social Work Supervisor. Participate in interdisciplinary planning, unit meetings, and Rusk evaluation conferences to coordinate treatment plans and ensure effective throughput with Care Managers. Mobilize community agencies, document activities per departmental protocols, and assist in orienting new staff or interns. Lead orientation patient/family groups to educate on diagnosis, treatment, and post-hospital care options.
Required Qualifications
- Masters Degree in social work, or its equivalent (e.g. a higher degree from a social work program that is registered by the New York State Education Department as qualifying for licensure), from a recognized college or university program accredited by the Council on Social Work Education (CSWE)
- New York State licensure as a Licensed Master Social Worker (LMSW) or Licensed Clinical Social Worker (LCSW) or a New York Education Department-issued limited permit
- Lic Clinical Social Worker
- Lic Social Worker - NYS-Permit
- Licensed Social Worker - NYS
- three years post-graduate
- SIFI eligible
- able to effectively communicate with all levels of the organization
