Job Summary
Provide PrEP and PEP education, navigation services, and case management for individuals seeking HIV prevention and treatment. Assist clients by obtaining medication, supporting appointment adherence, reviewing benefits, and facilitating enrollment in financial support programs. Track client progress, make referrals for support services, and provide linkage to care for those newly diagnosed with HIV or STIs. Maintain accurate documentation, communicate consistent with agency standards, and establish positive community relations across various settings including health fairs and partner organizations. Requires a flexible schedule involving evenings and weekends, on-site or hybrid work, and a valid driver's license.
Required Qualifications
- Excellent verbal and written communication in English
- Current BLS certification obtained through the American Heart Association or American Red Cross
- Ability to lift 24 pounds
- Current and valid driver's license
- Current personal auto insurance
- Access to patient protected health information required

Desired Qualifications
- Bachelor's degree preferred
- Knowledge of HIV disease, Sexually Transmitted Infections (Gonorrhea, Chlamydia, Syphilis) and HCV, to include diagnosis, prevention, risk-reduction strategies preferred
- Previous professional or volunteer experience working in HIV/STI/HCV prevention or care and/or delivering sexual health education preferred
- Understanding of how related chronic and infectious diseases are transmitted and prevented as well as familiarity with Pre-Exposure preferred
- Experience in medical or health-related fields; social work, counseling, nursing, public health or other health education preferred
- Ideal candidate will have interest in and competence in cultural differences
