Job Summary
Community Health Worker to connect patients with health, social, and supportive resources, provide sexual and reproductive health education, perform outreach and resource navigation, and document interactions in the EHR. Will travel across Contra Costa and Alameda Counties to support grant-funded Family Planning and reproductive health program activities, engage with community partners, and assist with program reporting and quality assurance activities. Bilingual English/Spanish preferred; role involves in-person outreach, partnerships, and coordination with healthcare providers and external agencies to improve access to care.
Required Qualifications
- Bilingual proficiency in English and Spanish or another language commonly spoken within the communities served
- Experience in community education, outreach, health promotion, resource navigation, or advocacy for community health and social service needs
- Excellent interpersonal, communication, and relationship-building skills
- Strong organizational and computer skills including proficiency with electronic documentation and data entry
- Ability to work independently and within a multidisciplinary team
- Ability to perform in-person outreach activities including standing for extended periods and walking during outreach
- Possession of a valid California Driver’s License and access to a personal vehicle for community-based work and travel

- TB clearance and a physical assessment by a physician for client/patient contact roles
- Immunization records including MMR, Hep B, Varicella, TDAP, Covid-19
- All positions require valid California driver’s license, listed as a driver on car insurance, and a clean driving record
