Job Summary
The HIMS Clerk performs clerical and filing duties in the Health Information Management Services (HIMS) area, handling inpatient and outpatient medical records, data entry, DRG/APC-related tasks, Daily Census, and assisting with Performance Improvement projects. Primary responsibilities include terminal digit filing, accurate chart routing and assembly, adherence to release of information procedures, management of Medical Record numbers and Master Patient Index, retention and confidential destruction of records, and processing information requests from patients and providers. The role requires organized, detail-oriented clerical work and knowledge of medical records workflows across inpatient and outpatient contexts.
Required Qualifications
- Knowledge of terminal digit filing system
- Accurately files completed charts
- Accurately routes incomplete charts
- Follows Release of Information policy and procedure for processing requests for health information
- Processes requests for health information from patients, healthcare providers, and others
- Retrieves and responds to requests from photocopy services
- Assigns new Medical Record numbers as needed
- Monitors and corrects computer Master Patient Index
- Responsible for purging of medical record charts to storage and implementation of retention procedure for appropriate and confidential destruction of records
- Keeps all filing current
- Can accurately assemble discharged patient records into correct chart order
- Keeps charts on the shelves in the aisles neat and orderly
- Creates medical record folders for new patients and on 3rd visit for Batch filed patients
