(TEMP) Care Coordinator - Sherman Oaks

Care Coordinator role overseeing coordination of care for ROWI clients, including orientation, transportation scheduling, doctor visits, and EMR documentation in a temporary full-time capacity. Responsibilities include ensuring program standards, supervising clients through programming, facilitating groups, conducting client orientation and assessments (e.g., PHQ9), ensuring compliance with Joint Commission Standards, documenting shift reports and transportation, managing lunch coordination, and maintaining professional interactions within an interdisciplinary team. Requires strong written and verbal communication, time management, ability to work with minimal supervision, and basic computer skills; driving duties included with a valid license and insurance eligibility. Minimum high school diploma; bachelor’s degree preferred; at least two years of driving experience and experience working with youth in a therapeutic setting are expected.
