Job Summary

Conduct telephonic outreach to close HEDIS gaps for the Medicaid pediatric population by evaluating member needs and coordinating services to improve pediatric health outcomes. Assist members in addressing barriers to care, including scheduling challenges and transportation needs, while providing vaccine awareness and education following Bright Futures and EPSDT preventive health guidelines. Collaborate with EPSDT Senior Quality Improvement Professionals and other departments to facilitate resource coordination and empower member independence in managing preventive and primary care needs. Perform basic administrative, clerical, and operational tasks within defined parameters, working remotely with two days of in-office presence.

Required Qualifications

  • Unrestricted Licensed Practical Nurse (LPN) in the state of Oklahoma
  • Remote/hybrid- 2 days a week in office
  • Prior experience in a insurance or health care setting
  • At least 2 years of technical experience
  • Comprehensive knowledge of Microsoft Office Word, Excel and PowerPoint

Desired Qualifications

  • Bilingual -Spanish
  • Experience working with Medicaid members to coordinate services, care needs, or benefits
  • Prior experience working with HEDIS measures or quality performance metrics
  • Remote or Telephonic experience
  • Knowledge of community health and social service agencies and additional community resources
