Job Summary
Telephonic Nurse Practitioner delivering telehealth/telephonic care for Optum Home & Community Care in a part-time, remote role. Provide virtual care to members across settings, perform comprehensive medical and behavioral assessments, document in EMR, coordinate care with providers and facilities, and participate in 24/7 coverage including holidays and weekends. Requires an active NP license in your state and Colorado, and MSN or DNP; roles involve evening/night/weekend shifts and cross-market coverage with strict privacy and CMS-compliant documentation.
Required Qualifications
- Active and unrestricted NP license in the state of residence and Colorado license; ability to obtain in other required locations
- New hires must be able to obtain collaborative practice agreement if applicable in state
- Graduate of an accredited Master of Science Nursing or Doctor of Nursing Practice program (MSN or DNP)
Desired Qualifications
- Active and unrestricted Nurse Practitioner license in the state of residence and Colorado license; ability to obtain license in other required locations
- MSN or DNP from an accredited program
