Job Summary
Telephonic Nurse Practitioner for Optum Home & Community Care. Remote full-time role providing telephonic/telehealth care to patients across multiple markets with 24/7 coverage, including evenings, nights, weekends, and holidays. Responsibilities include coordinating care between caregivers, facilities, and providers; delivering acute and chronic care management; conducting comprehensive assessments; documenting in EMR per CMS requirements; and participating in care coordination and quality initiatives. Requires NP degree (MSN or DNP), current NP license in home state plus eligible licenses in OH/MO/IN/WI as applicable, board-certified NP, DEA licensing, and ability to practice autonomously where permitted. Requires flexibility to cover varied shifts, strong communication with interdisciplinary teams, and adherence to patient privacy and regulatory standards.
Required Qualifications
- Graduate of an accredited Master of Science Nursing or Doctor of Nursing Practice program
- Active and unrestricted NP license in the state of residence and in Ohio, Missouri, Indiana or Wisconsin (and ability to obtain in other required locations)
- Board certification as NP (Family Nurse Practitioner, Adult Nurse Practitioner, Gerontology Nurse Practitioner, or Adult-Gerontology Acute Care NP)
- Current, active DEA licensure/prescriptive authority or ability to obtain post-hire
- Ability to gain collaborative practice agreement, if applicable in your state
- NDA/ autonomous practice authorization as applicable in jurisdiction (if applicable)
- Access to telephonic/telehealth platform and EMR proficiency
- Must meet state requirements for autonomous practice where applicable
