Job Summary
Telephonic Nurse Practitioner in a remote telehealth role delivering 24/7 care coordination and/or acute care to patients via telehealth/telephonic platforms. Requires NP with MS or DNP and an active, unrestricted license in the residing state plus ability to obtain licenses in Ohio, Missouri, Indiana or Wisconsin. Responsibilities include conducting telephonic/telehealth assessments, utilizing EMR, coordinating care across caregivers, facilities, and providers, and documenting visits in a CMS-compliant manner. Full-time, 40 hours/week with varied shifts including evenings, nights, weekends, and holidays; sign-on bonus for external candidates.
Required Qualifications
- NP: Graduate of an accredited Master of Science Nursing or Doctor of Nursing Practice program
- Active and unrestricted license in the state which you reside, as well as States of Ohio, Missouri, Indiana or Wisconsin, and ability to obtain in other required locations
- Active Nurse Practitioner certification through a national board: Family Nurse Practitioner, Adult Nurse Practitioner, Gerontology Nurse Practitioner, or Adult-Gerontology Acute Care Nurse Practitioner
- Current, active DEA licensure/prescriptive authority or ability to obtain post-hire, per state regulations (unless prohibited in state of practice)
