Job Summary
Telephonic Nurse Practitioner role delivering telehealth/telephonic care from a remote, home-based setting with 24/7 coverage across multiple markets. Responsibilities include taking and placing calls to coordinate care among members, caregivers, facilities and providers; using EMR for acute care documentation; delivering cost-effective, quality care for medical and behavioral/chronic/acute conditions in collaboration with physicians and specialists; coordinating transitions of care and ensuring appropriate documentation and CMS-compliant records; and participating in ongoing education and quality initiatives while maintaining excellent customer service in a virtual, interdisciplinary environment.
Required Qualifications
- Education: NP - Graduate of an accredited Master of Science Nursing or Doctor of Nursing Practice program
- Active and unrestricted license in the state of residence and in New York, New Jersey, Connecticut, Maine, New Hampshire, or Rhode Island (with ability to obtain in other required locations)
- Ability to gain a collaborative practice agreement, if applicable in your state
- APCs working in jurisdictions that authorize autonomous practice must have approval to practice autonomously or without supervision from licensing board (as applicable) and must apply to meet autonomy requirements per hire timelines
- Active Nurse Practitioner certification through a national board with Family Nurse Practitioner OR Adult Nurse Practitioner OR Gerontology Nurse Practitioner OR Adult-Gerontology Acute Care Nurse Practitioner
- Current, active DEA licensure/prescriptive authority or ability to obtain post-hire, per state regulations (unless prohibited)
- Ability to work scheduled shifts in accordance with scheduling policies
- Proficient computer skills and electronic medical records documentation
