Job Summary
Care Navigation LPN on-site role in Cleveland, OH supporting referral intake, care coordination, and clinical triage to ensure timely access to hospice services. Responsibilities include processing referrals from multiple sources (CarePort, electronic fax, phone referrals), scheduling appointments via dispatch, following up on referrals, performing direct admits to inpatient centers as needed, handling clinical calls from patients and caregivers, communicating with primary care teams, and participating in quality/process improvement initiatives. Requirements include an LPN license in Ohio, hospice experience preferred, strong communication skills, comfort with EMR and referral/intake platforms, and an active driver’s license. The position is non-exempt and on-site in Cleveland with a focus on compassionate, coordinated care.
Required Qualifications
- Licensed Practical Nurse (LPN) licensed in Ohio
- Hospice experience strongly preferred
- Excellent verbal and written communication skills
- Ability to calmly and compassionately communicate with patients and families in distress
- Strong teamwork and collaboration skills
- Active driver’s license
- Provide proof of eligibility to work in the United States
- Experience with Electronic Medical Records (EMR) and phone/referral intake platforms
