The Care Navigation Nurse LPN supports referral intake, care coordination, and initial clinical triage to ensure timely, accurate, and efficient access to hospice services. The role emphasizes one-call resolution whenever possible and seamless progression of referrals through dispatch. Responsibilities include retrieving and processing referrals from multiple sources, learning the dispatch process to schedule appointments for prospective patients, following up on referrals, performing direct admits to inpatient care centers as needed, handling clinical calls from patients and families, notifying primary care teams of clinical issues, and participating in quality and process improvement initiatives. Required skills include LPN licensure in Ohio, hospice experience preferred, strong written and verbal communication, calm and compassionate interaction with patients and families, teamwork, and proficiency with EMR and referral/intake platforms.
