Telemedicine palliative care Nurse Practitioner/Physician Assistant in California: provide remote palliative care to California-based patients post-hospital discharge. Responsibilities include assessing patients in a telemedicine environment, delivering palliative care, defining and documenting patient diagnoses, ensuring patient satisfaction, educating patients and families, recording findings and treatment courses in charts with follow-up notes, and participating in departmental activities to improve training methods and achieve company goals.
