Job Summary
Triage patients using protocols to determine nurse interventions and escalation levels. Provide virtual nursing assessments to evaluate symptoms, medication adherence, and disease progression. Monitor vital signs and survey responses to identify condition changes, then document interactions and interventions. Answer, direct, and follow up on incoming calls while recognizing urgent situations to seek medical provider assistance. Empower patients and caregivers through education on heart failure medications, adherence, and side effects. Work remotely from a HIPAA-compliant environment Monday through Friday from 8:00 AM to 4:30 PM PST.
Required Qualifications
- Full-time (FT) RN
- Specializing in cardiology and heart failure
- Bachelor's Degree in Nursing
- Minimum of 3 years experience in cardiology, heart failure management, or critical care nursing
- Strong background in heart failure patient management, including knowledge of GDMT (Guideline-Directed Medical Therapy)
- Current license to practice as a Registered Professional Nurse in a compact state and the state of California
- Experience with patient phone triage either in-office or via telemedicine
- Ability to learn and proficiently use multiple EMRs and various communication applications, including Google products
- Must be comfortable with the virtual use of episodic vital sign monitoring devices
- Ability to work from home in a HIPAA compliant environment
- Fluency in English and Spanish
- Some holidays may be required
Desired Qualifications
- Bilingual candidates (English and Spanish speaking) are highly preferred
- Advanced certification highly preferred (CCRN, CHFN, RN-BC)
- Additional licenses preferred: NY, IL, FL, MO, KY, RI
