Job Summary
Initiate preliminary TRRs and recovery plans for all consumers seen in triage within 7 days, establish new consumers, and facilitate transitions into the case management team. Provide rehabilitation case management or service coordination for persons with persistent and severe mental illness, including advocacy, crisis services, and individual or group skills training under trauma-informed and recovery-oriented models. Operate in accordance with CCBHC guidelines, Texas Administrative Code, and HHSC standards while supporting self-determination for individuals with mental and developmental disorders. Requires a Bachelor's degree in Social Work, Sociology, Psychology, or Counseling with two years of Human Services experience, a current driver's license, and proficiency in Microsoft Office.

- Bachelor's Degree in Social Work, Sociology, Psychology or Counseling
- two (2) years Human Services experience
- Current driver's license

- Ability to speak a second language relevant to population served preferred
- Must be self-motivated

- Ability to work with skill in identifying problems
- Will have access to confidential information abiding by the organization's privacy policies and regulations concerning this information
