Team RN, Hospice, Part Time, Evenings & Nights

Team RN, Hospice role performing routine nursing assignments in a hospice setting with responsibility for assessment, coordination and delivery of individualized nursing care, education for patients and families, and collaboration with an interdisciplinary team to implement an optimal plan of care while maintaining HIPAA/privacy standards and compliance with Sutter Health policies. Requires RN licensure in California, BLS certification, and a valid driver’s license; 2+ years of relevant hospice experience preferred. Shift includes evenings and nights on a part-time schedule; duties include direct patient care, care coordination, documentation in EHR, and ongoing professional growth. Equal opportunity employer.
