Job Summary
Lead and supervise the reablement social work team to deliver safe, strengths-based, person-centred practice; manage workflow with case allocation, performance monitoring, and timely assessments, reviews, and discharge planning in line with statutory duties; collaborate with health and community partners to enable seamless, outcome-focused reablement support across multidisciplinary teams.

- Full UK driving licence
- Post-qualified experience
- Degree level or equivalent in Social Work
