Job Summary
Manage the interdisciplinary team to provide comprehensive hospice care to patients and families, ensuring alignment with the Interdisciplinary Plan of Care. Coordinate daily operations, including visit management, productivity, and revenue targets, while overseeing staff hiring, training, and performance evaluations. Lead participation in team meetings, quality improvement initiatives, and regulatory audits to maintain compliance with Hospice Conditions of Participation. Analyze clinical, regulatory, and fiscal data to support department goals and budget development. Requires a Registered Professional Nurse license in New York State with minimum three years of hospice clinical experience. Daytime coverage only.
Required Qualifications
- License and current registration to practice as a Registered Professional Nurse in New York State
- Licensed and currently registered to practice as Nurse Practitioner in New York State and DEA license
- Bachelor's Degree in Nursing from an approved program accredited by the National League of Nursing or Health or Human Services field
- Minimum of three years clinical experience in hospice
- Effective oral, written and interpersonal communication skills
- Daytime coverage only
