Job Summary
Manage the interdisciplinary team to assess medical, physical, social, emotional, and spiritual needs of patients and provide support to families in accordance with the Interdisciplinary Plan of Care. Ensure regulatory compliance with Hospice Conditions of Participation through regular audits of files, processes, and procedures. Coordinate day-to-day operations, including visit management, productivity targets, and revenue/expenditure goals for the hospice care program. Lead quality improvement initiatives, staff training, performance evaluations, and hiring decisions while facilitating team meetings and committee work.
Required Qualifications
- License and current registration to practice as a Registered Professional Nurse in New York State
- Licensed and currently registered to practice as Nurse Practitioner in New York State
- DEA license
- Bachelor's Degree in Nursing from an approved program accredited by the National League of Nursing or Health or Human Services field
- Minimum of three years clinical experience in hospice
- Effective oral, written and interpersonal communication skills
- Daytime coverage only

- Knowledge of personal computer operations, including MS Office applications
