Job Summary
Manage the interdisciplinary team to provide hospice care to patients and families, ensuring implementation of clinical and quality improvement initiatives. Conduct audits, monitor files, and review processes to maintain full regulatory compliance with Hospice Conditions of Participation. Coordinate day-to-day operations, including caseload management, productivity targets, and fiscal goals. Evaluate staff performance, oversee training, and manage hiring and promotion activities. Participate in team meetings and committees to develop procedures and documentation tools. Complete assigned reports, projects, and special initiatives. Requires Registered Professional Nurse license in New York State.
Required Qualifications
- License and current registration to practice as a Registered Professional Nurse in New York State
- Licensed and currently registered to practice as Nurse Practitioner in New York State
- DEA license
- Bachelor's Degree in Nursing from an approved program accredited by the National League of Nursing or Health or Human Services field
- Minimum of three years clinical experience in hospice
- Effective oral, written and interpersonal communication skills
- Daytime coverage only

- Knowledge of personal computer operations, including MS Office applications
