Job Summary
Team Manager leading the Patient Care Team, directing and coordinating the delivery of comprehensive patient care services within the interdisciplinary team, ensuring continuity of care from admission to discharge or transfer, serving as patient advocate and coordinating with social service and health care providers in the community, maintaining patient records, managing the team schedule 24/7, facilitating team meetings, and supervising staff to meet quality, regulatory, and service goals.
Required Qualifications
- Current and valid RN License to practice in the state where the VITAS program is located
- Two years successful supervisory experience or equivalent in a health care organization
- Reliable transportation with appropriate license and insurance coverage for driver and passengers
- Bachelor's degree preferred
