Job Summary
Team Manager leads the Patient Care Team, supervising, evaluating and coordinating interdisciplinary team members; ensures continuity of care from admission to discharge or transfer; serves as patient advocate and coordinator with social services and providers; maintains patient records from intake through discharge; directs all patient care services including volunteer and bereavement; collaborates with team physicians on palliative measures; ensures timely recertification decisions; oversees staffing, scheduling, and 24/7 coverage; facilitates team meetings and participates in on-call rotation; mentors staff, manages payroll and budgets; ensures documentation quality and regulatory compliance; drives quality improvement and customer service initiatives; engages with LTC and contract-bed facilities to align care plans and satisfaction; requires RN license and Bachelor’s degree preferred, with supervisory experience in a healthcare setting.
Required Qualifications
- Reliable transportation with appropriate license and insurance coverage for driver and passengers.
- Private telephone in home.
- Knowledge of the principles and practices of primary medical and nursing care, with at least a working knowledge of oncologic nursing and palliative care.
- Working knowledge of VITAS information system (Vx) and the ability to utilize management reports.
- Two years successful supervisory experience or equivalent in a health care organization.
