Job Summary
Team Manager Home Care leads the Patient Care Team, supervising and coordinating the interdisciplinary care from admission to discharge, serving as patient advocate and coordinator for social services and health care providers. Directs all patient care services for the team, reviews admissions and discharges, collaborates with physicians on palliative care measures, ensures complete and accurate documentation for recertification, manages team scheduling and on-call rotation, participates in quality improvement and regulatory compliance activities, and supports payroll and productivity management within the hospice program. Requires RN license, relevant supervisory experience, knowledge of hospice/palliative care, and experience with the VITAS information system (Vx). Bachelor’s degree preferred.
Required Qualifications
- Current and valid RN license to practice in the state of the VITAS program location
- Bachelor's degree preferred
- Two years of successful supervisory experience or equivalent in a health care organization
- Knowledge of the principles and practices of primary medical and nursing care with working knowledge of palliative care
- Working knowledge of VITAS information system (Vx) and ability to utilize management reports
- Reliable transportation with appropriate license and insurance coverage for driver and passengers
- Private telephone in home
