Team Manager Home Care

Team Manager of the Patient Care Team leading home care operations: supervise, evaluate and coordinate the interdisciplinary team; ensure continuity of care from admission to discharge or transfer; advocate for patients and coordinate with social services and community providers; direct all patient care services including bereavement and volunteer support; manage staffing, scheduling, on-call rotation, and ensure timely, accurate documentation and data for recertification; collaborate with the team and physicians on palliative care measures; ensure adherence to regulatory standards, quality improvement initiatives, and patient/family satisfaction; mentor staff, oversee payroll and expenses, monitor utilization and cost effectiveness, and participate in program reviews; requires RN licensure and knowledge of primary medical and nursing care, hospice principles, and VITAS systems; two years supervisory experience in healthcare; bachelor’s degree preferred.
