Job Summary
Lead the Patient Care Team in a home care setting, supervising and coordinating interdisciplinary team members to ensure integrated care from admission to discharge (or bereavement transfer). Direct patient care services, review admissions and discharges, collaborate on palliative care measures, ensure complete and timely documentation for recertification, manage team scheduling and on-call coverage, facilitate team meetings, mentor staff, monitor quality improvements, and actively participate in regulatory and customer-service activities. Requires RN licensure, a Bachelor's degree (preferred), and supervisory experience within health care; familiarity with VITAS systems and hospice principles is essential.
Required Qualifications
- Bachelor's degree preferred
- Current and valid RN License to practice in the state where the VITAS program is located
- Two years successful supervisory experience or equivalent in a health care organization
- Knowledge of the principles and practices of primary medical and nursing care, with at least a working knowledge of oncologic nursing and palliative care
- Working knowledge of VITAS information system (Vx) and the ability to utilize management reports
- Reliable transportation with appropriate license and insurance coverage for driver and passengers
- Private telephone in home
