Job Summary
Team Leader role (0.5 FTE) and Senior Clinician (0.5 FTE) for the Mobile Support and Treatment Service based at Hawthorn, Melbourne. Lead daily operations, develop rosters, chair clinical reviews, participate in weekly intake discussions for case-management suitability, provide line supervision and mentorship to junior clinicians, and contribute to senior clinical leadership meetings. Drive comprehensive care planning with consumers and families, strengthen partnerships across inpatient, ED and community services, and ensure compliance with activity-based funding. Deliver targeted mental health interventions for complex cases, and maintain meticulous documentation (contacts, phase of care, outcomes, ICD-10 coding). Requires professional registration (AHPRA) in OT/SW/Nursing as applicable, and a relevant degree (OT, SW, or Nursing) with eligibility for professional membership. Required Qualifications
- Registration with the Australian Health Practitioner Regulation Agency (AHPRA)
- Undergraduate Degree in Occupational Therapy OR Masters of Clinical Practice in Occupational Therapy
- Social Worker degree and eligibility for full membership of the Australian Association of Social Workers (AASW)
- Nursing: postgraduate diploma or specialist mental health nursing program and registration with AHPRA
- Valid Nationally Coordinated Criminal History Check or willingness to obtain
- Experience: minimum of 7 years post-qualification clinical experience in Mental Health (preferred)
