Lead and coordinate daily hospital discharge activity and oversee discharge workflow for adults, supervising social workers and support staff; liaise with NHS teams, therapy services, and community partners to resolve delays and maintain multidisciplinary communication. Requires Social Work England registration, UK work eligibility, a full UK driving licence, post-qualification experience, and a degree in Social Work.
