Job Summary
Lead the Mental Health and Addictions Hub program by overseeing day-to-day clinical operations and patient flow, serving as the clinical resource for complex cases, guiding service coordination with health partners, supporting clerical workflow, driving quality improvement and evidence-based practice, and providing onboarding and mentoring to staff. The role requires strong collaboration with internal/external stakeholders, incident management and escalation, and representation at committees to advance coordinated access and timely care delivery. The position involves travel between sites (2-5 times per month) and requires demonstrated capabilities in change management, conflict resolution, and cross-sector relationship building; a Bachelor’s degree in a social service/healthcare field is required with Master’s preferred, and registration as a regulated healthcare professional is preferred.
Required Qualifications
- Bachelor’s degree in a social service/healthcare program or equivalent
- Master’s degree preferred
- Regulated Healthcare Professional (Registered Nurse, Social Work, Occupational Therapy) preferred
- Minimum of 5-7 years mental health and addiction service delivery experience in hospital and community setting
- Minimum 3 years working experience in Mental Health and Addiction coordinated access model
- Minimum of 3-5 years participating and/or leading clinical quality improvement projects
