TCM Coordinator

Review daily cases for admissions and discharges for inpatient stays and ER visits. Conduct post-discharge follow-up calls within 24–48 hours to review medications and report concerns to the clinic nurse. Schedule TCM appointments with primary care providers within five days of discharge. Identify and escalate clinical concerns to appropriate care providers. Access hospital portals to track admissions and retrieve discharge summaries, then document all patient interactions in the electronic health record. Collaborate with care teams to ensure continuity of care and assist with quality improvement initiatives related to readmission reduction. Maintain up-to-date knowledge of discharge protocols and transitional care workflows.
