Job Summary
Coordinate integrated services to assist adults recovering from mental illness toward recovery; partner with clients to develop Individual Community Support Plans or Individual Family Community Support Plans; provide collaboration and education to significant others and family members; ensure services are implemented as planned and continuously monitored, with documentation of client status changes; participate in staff meetings and trainings; role supports multilingual language proficiency (Spanish, Hmong, Somali) as part of delivering culturally-responsive care.
Required Qualifications
- Bachelor’s degree in social work, psychology or human services related field from an accredited college or university
- At least 2000 hours or more of supervised experience in the delivery of mental health services to adults
