Job Summary
Provide mental health targeted case management services to assist recipients in accessing medical, social, educational, and other services; coordinate treatment and support to optimize functioning; perform assessments (bio-psychosocial, comprehensive, medical questionnaires), document in Avatar within timelines; conduct home visits to assess safety; develop individualized service plans with measurable goals and guardianship when applicable; assist with accessing financial/insurance benefits, employment, housing, education, and other services; ensure service plan updates and CSB/CDM compliance; document progress notes within 24 hours; identify high-risk clients and conduct suicide risk assessments; collaborate with community agencies; participate in staff meetings; required CBHCM certification or provisional status within specified timelines; roles may require driver’s license and designated auto-driver status; adherence to Florida-specific certification requirements and client-handling standards.
Required Qualifications
- Certified Behavioral Health Case Manager (CBHCM) verified through the Florida Certification Board (FCB) per Florida Statute 394.4573(e) OR Bachelor’s Degree in counseling; Social Work; Psychology; Criminal Justice; Nursing; Rehabilitation; Special Education; Health Education; or a Human Services Field with relevant study; Must obtain CBHCM – Provisional Status (CBHCM-P) within 3 months of hire and CBHCM within 12 months OR Other Bachelor’s degree with three years full-time or equivalent experience working with children experiencing serious mental illness; Current valid driver’s license; Maintain Apalachee Designated Auto Driver status; Maintain current ADM Targeted Case Manager Certification.
