Job Summary
Impart agency philosophy of equal opportunity, independence, and individual potential to children, families, program staff, and community resources. Identify needs of disabled individuals and their families, then provide information, referral, and advocacy services to individuals of all ages with disabilities. Coordinate implementation of services by assuring communication and cooperation among all supports and providers, while arranging clinical assessments to determine functional status and intervention recommendations. Develop and coordinate components of individualized service plans, reviewing outcomes consistent with applicable regulations. Establish cooperative working relationships with area human service agencies, local school districts, and individuals as pertinent.
Required Qualifications
- Bachelor's degree
- Associate's degree
- Licensed Master Social Worker (Licensed by the New York State Education Department)
- Licensed Clinical Social Worker (Licensed by the New York State Education Department)
- Individual with a Doctorate or Master of Social Work
- Individual with a Doctorate or Master of Psychology
- Licensed Physical Therapist (Licensed by the New York State Education Department)
- Registered Professional Nurse (Licensed by the New York State Education Department)
- Certified Teacher of Students with Disabilities (Certified by the New York State Education Department)
- Certified Rehabilitation Counselor (Certified by the Commission of Rehabilitation Counselor Certification)
- Licensed Speech Pathologist (Licensed by the New York State Education Department)
- Licensed Occupational Therapist (Licensed by the New York State Education Department)
- Valid NYS driver's license
- Knowledge of community resources, social service agencies and means of accessing services
- Must be able to lift 10 pounds
- Must be able to lift 30 pounds occasionally
