Job Summary

Provide life coaching and case management services to Transition Age Youth in South Los Angeles, developing individualized housing plans to secure permanent housing and self-sufficiency. Meet with youth weekly to monitor progress, complete clinical assessments, and coordinate crisis interventions while assisting with education and vocational support. Utilize a personal vehicle to transport youth to appointments and help identify housing options through credit checks and referrals. Perform recordkeeping on electronic systems like HMIS, report issues to supervisors, and attend staff meetings. Requires a valid California driver's license, TB clearance, and background screening.

Required Qualifications

  • Bachelor's degree from an accredited college or university in social work, human services or a related field
  • Minimum of two-years experience providing services to persons experiencing homelessness
  • Demonstrated experience in working with at-risk youth to reach their housing, employment, or education
  • Demonstrated experience in one or more of the following areas: mental health, chemical dependency, emotional/behavioral special education, at-risk youth
  • Demonstrated knowledge of and ability to work effectively with youth with a variety of identities, cultures, and backgrounds including ethnic/racial minorities and LGBTQ
  • Knowledge of Trauma-Informed Care and Practices
  • Strong written and verbal communication
  • Creative problem-solving
  • Exceptional organizational
  • Valid California Driver's License
  • Proof of auto
  • TB Test Clearance
  • Successful completion of background screening
  • Ability to work with diverse population
  • Proficiency in MS Office Suite (Word, Excel, PowerPoint, Outlook)
  • Experience using a data management system to record client information

Desired Qualifications
