Job Summary
Administer psychosocial screenings/assessments and coordinate linkage to behavioral health, medical, educational, and vocational services for the justice-involved population. Provide ongoing professional care management while collaborating with community partners, legal systems, healthcare providers, and social services. Manage caseloads independently using electronic health record systems and remote collaboration tools like Teams and Zoom. This role supports the TASC (Treatment Accountability for Safer Communities) program at Coastal Horizons Center, serving individuals in Craven and Pamlico Counties.
Required Qualifications
- Bachelor's Degree or higher in a human services or related field
- Knowledge of recovery, harm reduction, and wellness-based concepts
- May have lived experience with recovery and/or the justice system
- Strong communication, organizational, and time-management skills
- Ability to work independently and manage a caseload effectively
- Proficiency in the Microsoft Office Suite and similar platforms
- Proficiency in using electronic health record systems and remote collaboration tools (i.e., Teams, Zoom, etc.)
- Demonstrated ability to collaborate with diverse stakeholders, including legal systems, healthcare providers, and social services
Desired Qualifications
- North Carolina Peer Support Specialist certification
- Minimum of 2 years of experience
- Registered with the North Carolina Addiction Specialist Professional Practice Board (NCASPPB)
