Job Summary
Provide TASC care management within the TASC program for justice-involved individuals; administer psycho-social screenings/assessments; coordinate linkage to behavioral health treatment as well as medical, educational, and vocational services; provide ongoing professional care management services for the justice-involved population; collaborate with community partners and justice systems to help individuals access treatment and recovery services; manage a caseload independently; proficient in EHR systems and remote collaboration tools (Teams, Zoom); strong communication, organizational, and time-management skills.
Required Qualifications
- Education: Bachelor's Degree or higher in a human services or related field
- Certifications: Preference given to North Carolina Peer Support Specialist certification and/or NCASPPB registration
- Experience: Knowledge of recovery, harm reduction, and wellness-based concepts; may have lived experience with recovery and/or the justice system
- Skills: Strong communication, organizational, and time-management skills; ability to work independently and manage a caseload; proficiency with Microsoft Office Suite
- Proficiency in using electronic health record systems and remote collaboration tools (e.g., Teams, Zoom)
- Ability to collaborate with diverse stakeholders, including legal systems, healthcare providers, and social services
