Adult Case Manager, BHCM working with adults with mental health or co-occurring disorders in the community; supports linkage, coaching, and guidance, coordinates financial support, housing, transportation, and utilizes community resources; field-based role within a collaborative care team led by a Department of Health provider; requires a Bachelor's in Social Work and two years of relevant experience; located in Clearwater, FL.
Required Qualifications
- Bachelor's Degree in Social Work or a related area of study from an accredited university
- Two years of experience working with the homeless population and/or persons struggling with severe and persistent mental illness and/or substance misuse disorders
