Job Summary

Targeted Childrens Case Manager role coordinates access to medical, social, educational and other services for clients with complex needs to optimize functioning and independence. The role requires delivering individualized, integrated mental health, substance abuse and co-occurring services, traveling locally to clients’ homes and training locations, and having knowledge of local resources. Requires a Bachelor’s degree in a related human services field and either 1 year of relevant experience or 3 years with a Bachelor's, plus ability to complete approved targeted case management training within three months, adherence to applicable state/federal statutes and policies, and successful background screening. A valid driver’s license and insurability under the corporate policy are also required.

Required Qualifications

  • Bachelor’s degree from an accredited university or college with a major in counseling, social work, psychology, criminal justice, nursing, rehabilitation, special education, health education, or a related human services field
  • Minimum of one (1) year of full time or equivalent experience working with children with serious emotional disturbances OR Bachelor’s degree and three (3) years full time or equivalent experience working with children with serious emotional disturbances
  • Ability to complete approved mental health targeted case management training within three months of initially providing case management services
  • Knowledge of available resources in the service area for children with serious emotional disturbances
  • Knowledge of and ability to comply with state and federal statutes, rules and policies that affect the target population
  • Ability to provide individualized, integrated mental health, substance abuse and co-occurring services to meet the needs of individuals served and their families
  • Ability to travel locally to client’s homes, training locations and other locations as necessary
  • Must pass organization screening requirements, including state or federal background screenings as appropriate
  • Must possess a valid driver’s license from the state in which you reside, a good driving record and be insurable under the corporate policy
