Job Summary
Provide support for the wellness and recovery of clients with complex behavioral health issues through targeted interventions and psychosocial rehabilitation. Manage referrals and linkage to community services while assisting clients in overcoming behavioral barriers to independent community functioning. Advocate for family support and required services through management and monitoring processes for forensically connected adults with mental health or substance abuse challenges. Requires experience working with child, adolescent, and adult populations. Schedule includes full-time hours at the Rochester clinic location.
Required Qualifications
- Bachelor's degree in a Human Services field
- Displayed experience in a similar position
- Excellent communication skills
- Interest in working with a diverse population of adults with mental health and/or substance abuse issues including the forensics population
- Ability to work with child, adolescent, and adult clients
- Ability to verify identity and eligibility to work in the United States
- Ability to complete the required employment eligibility verification form upon hire

- Electronic health records software experience (Cerner)
