Job Summary
Targeted Case Manager Level I, Full Time Exempt, located in Oakland County, MI, providing targeted case management within a Certified Community Behavioral Health Clinic (CCBHC). Responsibilities include assisting individuals in sustaining mental health and substance use recovery and helping them access medical, behavioral health, social, educational, legal, and community services; connecting clients to services to resolve barriers to independence; working Monday–Friday 8:30am–5:00pm. Requires a bachelor’s degree in social work, psychology, nursing, human services, or related field with state licensure/registration as an RN, and at least one year of direct client service experience; experience at a CCBHC/FQHC preferred. Required Qualifications
- Bachelor's degree in social work, psychology, nursing, human services, or related field including state licensures and registration as an RN
- At least one year of direct service experience with mental health and/or substance use disorders
- Foundational knowledge of behavioral health systems and community resources
- Experience at a CCBHC or Federally Qualified Health Center preferred
