Targeted Case Manager I

Assess patient eligibility for specific service needs and develop individualized integrated action plans for Substance Use Disorder patients. Refer and connect individuals to medical, social, education, and employment resources while monitoring adherence and providing necessary follow-ups. Complete detailed reports on needs assessments and ensure timely input of all case management data into the electronic patient health record. Provide crisis and de-escalation techniques and ensure patients have reliable transportation to care providers. Attend required supervision, treatment team meetings, and quarterly in-service sessions while adhering to regulatory requirements. Maintain a high degree of integrity and compliance with work standards and HIPAA protocols.
