Job Summary
Complete Social Determinants of Health (SDOH) assessments and participate in the Open Access process to assist patients with rapid care. Provide patient support via outreach calls, telehealth, and face-to-face visits to identify barriers and connect individuals to medical, social, legal, and housing resources. Deliver transitional care by following up with patients from inpatient, emergency, or residential settings to ensure continuity. Retrieve and utilize clinical data from internal Electronic Health Record (EHR) systems and regional health information networks. Build positive relationships with individuals who have behavioral health disorders and their families while adhering to privacy and confidentiality regulations. Monday through Thursday: 8am - 5:15pm, Friday: 8am - 1pm. Salary range $20.00 - $24.00. Bachelor's degree and six months of behavioral health experience required, or associate degree with one year, or high school diploma with two years. Located at 637 Davison Road, Lockport NY.

- Computer navigation skills
- proficiency in Microsoft office skills
- Local travel between worksites
- Handling light duties, occasionally lifting objects up to 20 pounds
- Exposure to various environmental conditions including low/high temperatures, outdoor elements
- Monday through Thursday: 8am - 5:15pm, Friday: 8am - 1pm
Desired Qualifications
- Experience working within Electronic Health Record (EHR) systems and willingness to learn new software applications preferred
- Familiarity with reporting and entry level data analytics
