Job Summary
Coordinate Medicaid benefits, screen eligibility, and assist individuals in maximizing community integration. Provide crisis intervention, teach skills, and advocate for services while navigating complex health and social systems. Monitor progress through annual contacts and collaborate with families, providers, and entities to ensure continuity of care. Prepare consumer records and develop services when current options are inadequate. Report emergencies to leadership and adhere to agency policies. Full-time role with minimal travel.
Required Qualifications
- High school diploma or equivalent
- Must be able to work full-time (40 hours per week)
- Must be able to work a schedule determined by supervisor
- Must be able to work variable hours depending on Company need and workload
- Must be able to receive notice when schedule changes occur
- Must be able to travel minimal distances
- Must be able to adhere to agency policies and procedures
- Must be able to comply with all CABHS policies and procedures
Desired Qualifications
- Work with mental health or substance use populations preferred
