Targeted Case Manager

Assess patient biopsychosocial stressors and develop strategies to address housing, financial, legal, and medical needs during and after treatment. Coordinate care among multiple service providers for inpatient and outpatient settings, facilitating case coordination, information sharing, aftercare planning, and court-related issues. Maintain positive relationships with support networks, government resources, employers, and referral sources while completing FMLA and Short-Term Disability documentation. Review and complete case notes, prepare patient discharges by coordinating community services, and participate in weekly multidisciplinary team meetings. Manage daily census, coordinate discharge schedules, and transport patients to Intensive Outpatient Programs. Maintain professional licensure through required Continuing Education and meet all applicable State Regulations.
