Targeted Case Manager - Adult Outpatient Services

Job Summary
Targeted Case Manager provides direct case management for adults (and potentially children/families) with mental health and substance use disorders, including case management assessment, person-centered planning, referral/linkage, monitoring, and follow-up. Maintains caseload per CIHA policy; documents treatment events in the EHR; coordinates discharge planning, follow-up care, and transfers; collaborates with medical staff and community providers; conducts home visits as needed; participates in clinical staffing and program improvement; engages with schools, courts, probation, and other agencies; supports patient and family through crisis intervention and treatment planning, and adheres to confidentiality and HIPAA requirements. Strong knowledge of crisis intervention, engagement techniques, and the Cherokee culture; ability to travel and work in hospital/clinic and community settings.
Required Qualifications
- Qualified Professional (QP) within MH/SA system or equivalent—license/provisional license/certificate/registration/permit with four years of MH/SA experience; or Master's degree in a human service field with one year of full-time MH/SA experience; or Bachelor's degree in human services with two years of MH/SA experience; or Bachelor's in another field with four years MH/SA experience; Valid drivers' license; Specific experience working with Native Americans preferred.
