Job Summary
Targeted Case Manager (TCM) provides case management services to individuals in programs addressing severe mental illness, substance use, and/or serious emotional disturbance. Responsibilities include assessing client/family strengths and needs; linking to community resources; advocating with external services; maintaining regular contact (minimum 4 contacts/month including 2 face-to-face); collaborating with Lifeskills and external providers to support recovery; ensuring client access to resources; delivering services in clinic and community settings; facilitating client engagement in meaningful community roles; transporting clients using a personal vehicle; providing billable targeted case management to 23–25 clients per month; and performing occasional coverage as needed, with timely documentation. Required Qualifications
- Bachelor of Arts or Science degree in a behavioral science field (e.g., Psychology, Sociology, Social Work, Family Studies, Human Services, Counseling, Nursing, Behavioral Analysis, Public Health, Special Education, Gerontology, Recreational Therapy, Education, Occupational Therapy, Physical Therapy, Speech-Language Pathology, Rehabilitation Counseling, or Faith-Based Education)
- One year of post-graduate experience working with individuals with mental health, SUD, or SED; master's degree in a behavioral science may substitute for the required experience
- Reliable transportation with driver’s license and auto insurance to transport clients
- Strong written and verbal communication skills
