Job Summary
As a Care Manager, you'll deliver a full range of care management services including comprehensive assessments, individualized care plans, care team development, ongoing coordination, transitional care, health promotion, medication monitoring, addressing unmet resource needs, and 24/7 on-call crisis support. The role combines in-person visits and phone-based support in a flexible, hybrid schedule. Requires a Bachelor's degree in a human services field (or RN licensure) and at least 2 years of relevant experience; valid driver's license and reliable transportation are required. Required Qualifications
